Tadalafil is a selective inhibitor of PDE5. PDE5 is found in the smooth muscle of the corpus cavernosum, prostate, and bladder as well as in vascular and visceral smooth muscle, skeletal muscle, urethra, platelets, kidney, lung, cerebellum, heart, liver, testis, seminal vesicle, and pancreas.

Penile erection during sexual stimulation is caused by increased penile blood flow resulting from the relaxation of penile arteries and smooth muscle. This response is mediated by the release of nitric oxide (NO) from nerve terminals and endothelial cells, which stimulates the synthesis of cGMP in smooth muscle cells. Cyclic GMP causes smooth muscle relaxation and increased blood flow into the corpus cavernosum. The inhibition of phosphodiesterase type 5 (PDE5) enhances erectile function by increasing the amount of cGMP. Tadalafil inhibits PDE5. Because sexual stimulation is required to initiate the local release of nitric oxide, the inhibition of PDE5 by tadalafil has no effect in the absence of sexual stimulation.

Super Vikalis VX 80 mg administered to healthy men produced no significant difference compared to placebo in supine systolic and diastolic blood pressure and in standing systolic and diastolic blood pressure. In addition, there was no significant effect on heart rate.

Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) marketed for the treatment of premature ejaculation in men. Human ejaculation is primarily mediated by the sympathetic nervous system. The ejaculatory pathway originates from a spinal reflex centre, mediated by the brain stem, which is influenced initially by a number of nuclei in the brain (medial preoptic and paraventricular nuclei).

It is recommended that tablets be taken with at least one full glass of water.

Do not take Super Vikalis VX 80 mg if you are also taking:

• antiepileptics
• antidepressants
• antipsychotics
• anxiolytics
• sedative hypnotics

Do not take Super Vikalis VX 80 mg with recreational drugs such as: ketamine, methylenedioxymethamphetamine (MDMA), lysergic acid diethylamide (LSD).

Serious rare side effects

Although serious side effects are rare, if you experience any the following serious side effects, stop taking Super Vikalis VX 80 mg and seek emergency medical care immediately.

• chest pain
• dizziness during sex
• erection longer than 4 hours (Priapism)
• severe decrease or loss of vision
• severe decrease or loss of hearing
• shortness of breath or trouble breathing

If this serious problem occurs, stop taking Super Vikalis VX 80 mg and get medical help right away. You have a slightly greater chance of developing NAION if you have heart disease, diabetes, high cholesterol, certain other eye problems ("crowded disk"), high blood pressure, if you are over 50, or if you smoke.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including:

• rash
• itching/swelling (especially of the face/tongue/throat)
• severe dizziness
• trouble breathing

Patients taking the following medicine types should NOT take Super Vikalis VX 80 mg:

• nitroglycerin medicines
• nitroprusside medicines
• amyl nitrate (recreationally known as poppers)
• guanylate cyclase (GC) stimulators
• alpha-blockers (caution is advised when PDE5 inhibitors are coadministered with alpha-blockers)
• organ donor recipient medicines
• SSRI in combination with a monoamine oxidase inhibitor (MAOI)
• recreational drugs (ketamine, methylenedioxymethamphetamine (MDMA) and lysergic acid diethylamide (LSD))
• thioridazine
• serotonergic medicinal/herbal products (including MAOIs, L-tryptophan, triptans, tramadol, linezolid, SSRIs, SNRIs, lithium and St. John's Wort
• CNS active medicinal products (e.g., antiepileptics, antidepressants, antipsychotics, anxiolytics, sedative hypnotics)
• potent CYP3A4 inhibitors
• potent CYP2D6 inhibitors
• azole antifungal medicines taken orally (topical creams for treatment of candida/ thrush are safe)
• serious hypersensitivity to tadalafil
• antihypertensives
• antacids
• alcohol

Both alcohol and tadalafil, act as mild vasodilators. When mild vasodilators are taken in combination, blood-pressure-lowering effects of each individual compound may be increased. Substantial consumption of alcohol in combination with Super Vikalis VX 80 mg can increase the potential for orthostatic signs and symptoms.

Super Vikalis VX 80 mg is not suitable for everyone.

Patients under treatment for the following conditions; or any ongoing managed condition, should consult their doctor prior to taking Super Vikalis VX 80 mg:

• heart related & cardiovascular diseases including aortic stenosis, idiopathic hypertrophic subaortic stenosis and general ventricular outflow obstruction
• stroke within the last 6 months
• myocardial infarction (heart attack) within the last 90 day
• uncontrolled arrhythmias
• cardiac failure or coronary artery disease causing unstable angina
• blood pressure related diseases including hypertension & hypotension and patients with severely impaired autonomic control of blood pressure
• resting hypotension (low blood pressure) of (BP <90/50)
• resting hypertension (high blood pressure) of (BP >170/110)
• retinitis pigmentosa (genetic disease of the retina)
• sickle cell or other related anaemias
• kidney disease
• liver disease
• bleeding disorders
• active stomach ulcers
• HIV (specifically on treatment with protease inhibitors)
• organ donor recipients
• mania/hypomania or bipolar disorder

This drug may make you dizzy. Alcohol or marijuana (cannabis) can make you more dizzy. Do not drive, use machinery, or do anything that needs alertness until you can do it safely. Limit alcoholic beverages. Talk to your doctor if you are using marijuana (cannabis).

Patients should be advised not to use Super Vikalis VX 80 mg in combination with recreational drugs. Recreational drugs with serotonergic activity such as ketamine, methylenedioxymethamphetamine (MDMA) and lysergic acid diethylamide (LSD) may lead to potentially serious reactions if combined with Super Vikalis VX 80 mg. These reactions include, but are not limited to, arrhythmia, hyperthermia, and serotonin syndrome. Use of Super Vikalis VX 80 mg with recreational drugs with sedative properties such as narcotics and benzodiazepines may further increase somnolence and dizziness. If someone has overdosed and has serious symptoms such as passing out or trouble breathing, call doctor or emergency immediately.
